This exercise will show you what you currently have to offer an employer. It’s a reality check on the
education and experiences you have right now. We’ll use this as a starting point for taking a look at

your skills and abilities. In the next exercise we’ll really explore all your skills in detail, but for now,
this is the basic information every employer wants to know.

Information on a Job Application
The following information is usually included on a job application:
Personal Information– This includes your name, address, phone number, and social security
number. You will need to list your previous addresses. You will also be asked if you are eligible to
work in the United States and if you have been convicted of a felony. Answer honestly.
Position Desired– What job are you looking for? Do you want full-time or part-time work? When
can you start?
Employment History– List information about each of your last 3 to 4 jobs. List the company name,
address, phone number, and dates of employment. List your supervisor’s name. Give your job title,
the things you did on the job (what skills you used), how long you held the job, why you left the
job, and how much you were paid.
Education & Training– List all your education as well as any vocational education, training, or
certifications you have. Fill out the name of the school, the school’s address, what you studied, and
when you graduated.
Special Job-Related Skills– List any special skills you have that would be helpful on the job.
References– Most job applications also want the contact information for three people (not related
to you) who can tell them about you. Choose people who have good things to say about you and
your work habits. List their name, address, phone number, and position or job title. You can use
past supervisors, clergy members, teachers, counselors, and community leaders as references.
Instructions:
1. Complete the job application forms on the following pages. You’ll be using this information
later to look at your skills and create your résumé, so give as much detail as possible.
