Skill Level Assessment– indicates the current level of proficiency the individual displays
in specific skills.
Parent’s Description of why these skills are important–indicates the individual’s
understanding of the impact of gaining skills in this area.
Assessment Checklist—
Your signature means you have reviewed the
knowledge, skills, and abilities (KSA’s) with the
parent.
The parent’s signature means they could execute
the objectives. (In the case of low-functioning
parents, this may mean you delivered the
content through discussions using the discussion
questions and activities. For high-functioning
parents, they may be able to achieve the learning

objectives though reading and minimal
discussion.)

individual’s ability against the criteria described
on the signoff sheet and noted the current level
of skill integration.
The parent’s signature means they have
completed the pamphlet and agree with the
current level assessment.
How do I prove the parent completed the pamphlet?
Every document requires both a parent and provider/trainer signature. Time is provided
within each pamphlet session to debrief with the individual. Photocopies of activities
completed by the parent may be attached to the assessment documents.
What goes in the parent file?
An assessment document is available for each pamphlet. This assessment process was
designed to allow the parent to keep their original work and through review and summary
create an outcome assessment process with clear signoffs for the parent file.
