Assessment Documents in College Boot Camp
Initial Assessment– The first page of the pamphlet set provides an initial assessment
questionnaire containing 20 questions. Before using the pamphlet series, the individual
should take the one-page questionnaire. It should take 10-15 minutes to complete. The
second page of the assessment set provides a key for prioritizing pamphlet need based on
the responses of the individual. If individual instruction is available, this will give the
trainer insight into areas that may be more challenging to the learner, and more time be
given to these topics.
Assessment Checklist–Each College Boot Camp session has
an outcomes checklist, designed to itemize the measurable
components of the session. Each assessment can be signed by
both the instructor and the student.

Each form consists of three parts:
Assessment Checklist–identifies the key knowledge, skills,
and abilities the individual should be able to demonstrate
after completing the pamphlet and the IM exercises.
Skill Level Assessment– indicates the current level of
proficiency the individual displays in specific skills.
Assessment Checklist—
Your signature means you have reviewed the KSAs with
the individual.
The individual’s signature means they could execute the
objectives. For Evaluation Only
How do I prove the student completed the session?
The assessment documents required for completion are identified at the beginning of each
session. Every document requires both an individual’s and instructor’s signature. Time is
provided within each pamphlet session to debrief with the individual. Photocopies of
activities completed by the student may be attached to the assessment documents
and included in the individual’s file.
Grading Rubrics
Grading rubrics for the College Prep Portfolio Workbook are included at the back of the IM.
Use these as a guideline for assigning grades to specific exercises. We assigned five grading
levels to each exercise +++, ++, +, -, and --; equating to an A-F grading scale. Thoroughness
and extra effort are needed to achieve a +++ or ++ grade.
